Anatomy of the Tooth

The Tooth is a complex organ of mastication structured to help us in chewing, speaking, and swallowing. Every tooth has a basic composition and is shaped to perform specific masticatory function.

Structural Composition of a Tooth

In general the tooth has a crown portion which lies above the gums and the root portion lying underneath the gums and in the bone.

Tooth can be divided into three distinct sections namely, Enamel, Dentin and Pulp Chamber.

Enamel

The enamel is the outer most layer of the tooth which gives the tooth its white colour. Enamel is the hardest structure in the human body. It is comprised of prismatic crystalline structure called as Hydroxy-apatite crystals.

The thickness of enamel varies in different parts of the tooth but is thickest at the cuspal heights and incisal edges and thinnest at the junction of the crown and root called as the Cemento-enamel Junction(CEJ).
Enamel is thicker in children as compared to adults which gives the teeth a “milky white” appearance in children. The level of thickness of the enamel will determine the hue of the teeth as in the amount of “whiteness” of “yellowness” of the teeth in different individuals.

Dentin

Dentin is the next layer below the enamel and is yellowish in colour. It comprises of dentinal tubules and is porous compared to enamel. Each dentinal tubule is hexagonal in cross-section and extent from the Dentino-enamel junction (DEJ) to the pulp chamber. These dentinal tubules house the odontoblasts and their processes. These odontoblasts draw their nutrients from the pulp chamber.
Sensitivity in the tooth is felt by many people and the main reason for the sensation is because any cold or hot food or drink taken, will come in contact with the dentinal tubules and the shock will be sent down to the pulp chamber which holds all the nerve vessels and pain will be felt.

Pulp Chamber

This is the place where all the blood vessels and nerve vessels are present collectively called as pulp. The entire tooth gets its nutrition from the pulp. If the enamel is broken and the infection reaches the dentin then there is a very high chance for the infection to spread down to the pulp causing severe pain, swelling and even death of the tooth.

Sequence of dental caries

This is how a normal tooth looks like when it has been maintained well with proper brushing and oral care.

1. When one fails to keep the mouth clean by proper brushing, flossing and rinsing then food particles begin to accumulate on the tooth, leading to bacterial action and acid release and the first signs of tooth decay begins involving enamel - INITIAL DENTAL CARIES.

2. Once the infection spreads through out the enamel layer of the tooth and it not recognised and treated by the dentist, the infection spreads further into the dentin layer which may induce sensitivity in the underlying pulp- CHRONIC REVERSIBLE PULPITIS

3. As the infection spreads further, the infection reaches the pulp chamber, leading to severe pain and inflammation of the blood vessels and involves the nerves of the affected tooth. The tooth is now in a state of “no return”.- CHRONIC IRREVERSIBLE PULPITIS.

4. At this stage the tooth is completely damaged by the advancing bacterial infection and no amount of medication can help. The tooth has lost its strength and becomes brittle. The tooth must now be treated with Root Canal Treatment (RCT) or removed completely (Extraction), to prevent further spread of infection into the bone.- CHRONIC IRREVERSIBLE PULPITIS

5. This is the last stage of the disease where the underlying supporting bone also gets infected by the bacterial infection and there is puss accumulation in the bone and severe swelling and pain, associated with fever and lethargy (tiredness). CHRONIC IRREVERSIBLE PULPITIS LEADING TO SPACE INFECTION!

Gaps in between teeth?

Crowding and spacing are among the most common teeth problems in adults, with crowding affecting about 24% of women and 14% of men, and spacing found in 8% of women and 13% of men. In Asian society, ambivalence exists as to whether a diastema is a favorable physical trait or not. A small mid-line diastema is usually not considered unattractive by any group of people.

However, as the size of the gap increases to a more easily seen physical trait, the level of comfort someone has in personally possessing a “gapped-tooth appearance” may decrease to the point of creating a feeling of self-consciousness. If the condition develops an outward self-consciousness in relation to the way a person feels about a personal physical trait they possess, it can affect the way that person acts in life. This, in turn, could have a negative influence in certain career and social situations in which they are placed.
There can be wide variations of perception concerning the significance of a midline diastema when translated through different cultures, socioeconomic groups, and ethnic backgrounds. Indeed, unfair and ill-perceived judgments are leveled at people who possess certain physical traits. When testing the perceptions of subjects viewing anonymous faces with certain features, including dental midline diastemas, the subjects ranked the individuals in the images to be significantly less intelligent, beautiful, and sexually attractive. In addition, the anonymous faces were judged to belong to a lower socioeconomic class than the same faces with an ideal occlusal presentation. This finding illustrated that a social disadvantage was wrongly implied, simply on the appearance of one’s dentition—even if it was a healthy one.

Case Report

An 51-year-old asian  female reported to Dentist & Dontist Dental clinic, Hyderabad, with unsightly appearance of her teeth. On examination she had normal medical history. Coming to the dental history she had visited a dentist 5 years ago for checkup & he had suggested filling the gap in between her teeth. A composite restoration was done & it was fine for 2 years .the restoration was lost after 2 years  and she got it filled again but it was lost. Vexed with the filling she didn’t get it done again, but she was very unhappy with her smile.

Treatment, Progress & Results

The patient was seen every eight to 12 weeks (four to six aligners) to check for aligner fit, and cooperation (Fig. 2). Excellent patient compliance contributed significantly to the success of this case. The final result showed good alignment and occlusion, with up right canines and ideal overbite and overjet (Figs-3)., they compared favorably with many published cases treated with fixed appliances.

Mouth ulcer? Gone in just a minute!!!

Get relief from mouth ulcers in just a MINUTE. Yes, now it’s a reality!

Mouth ulcer can be cured in just a minute..no more suffering from long waiting..gone are the days when u have to keep applying gel for a week with no significant pain relief. With the most advanced techniques available at Dentist & Dontist, mouth ulcer can be healed in just a minutes time..u get immediate & instant pain relief.

What exactly are these mouth ulcers?

There are times when one may feel uneasiness like burning or painful sensations while eating and drinking due to mouth ulcers. So what are they exactly? Mouth ulcers, also known as canker sores, is a fairly common problem. It is essentially a painful sore that occurs in the mouth and can occur as a single sore or in clusters. It could appear on the tongue, the inside of the cheeks, gums or even on the lips. As for appearance, they are usually white or yellow colour and are
surrounded by red halos. They usually heal within a period of seven to 10 days.

Causes of mouth ulcer:

Vit c deficiency,poor oral hygiene,cheek biting,or excess stress may
result in mouth ulcers. They often seem start by themselves as a small bubble or blister (this stage is easy to not notice) which then becomes an open and ulcerated pit or crevass when the pain really starts. Alternatively they can be
started by trauma to the lining of the mouth (e.g. by accidently biting onesself).
The worst thing about mouth ulcers is the pain that they cause, which is constant, excruciating and made worse by eating, drinking and talking. The picture of the homunculus that you may recall from schooldays shows how very sensitive the mouth and tongue are compared to most other parts of the body - which explains the amount of discomfort caused by something so small.
Severe ulcers cause sufferers to actively avoid eating, drinking, talking and kissing, which can be awkward socially. Speech is painful resulting in a loss of clarity or enunciation - and sometimes spitting, which doesn't help. Sufferers often get worn down by the pain and become fatigued and depressed, and so I have been told, irritable too. No specific single cause has yet been isolated, though it seems they are not generally caused by infectious agents such as viruses or bacteria and are therefore not considered contagious.

Whatever may be the cause u get instant pain relief thanks to the
latest treatment techniques available at Dentist & Dontist
